Referral Templates

  • Medical Director Referral Template here - import to MD as per usual.

How do I refer a young person to Peninsula headspace?

Referrals can be for a general assessment, specific interventions or a combination of both.

The young person’s consent needs to be obtained before involvement.

Referrals can be received via ARGUS or faxed through.

Referrers are encouraged to discuss potential referrals with the intake workers.

Young people can also refer themselves by telephoning or dropping in.

What happens after referral?

Referrals received from General Practitioners will be either assessed by a Clinical Care worker (if it is a general referral) or linked in with a psychologist/psychiatric registrar (via a GP Mental Health Care Plan).

If you feel that a young person requires more support, they can be assessed for this as well as be linked in with a psychologist.

Peninsula headspace will liaise with the referrer and the young person’s current supports, and work with them to achieve the goals set out in the care plan.

Involvement is based on an episode-of-care model, and the allocated worker will be responsible for communicating with the care team.

What Peninsula headspace is not.

Peninsula headspace is NOT a crisis service – crisis referrals will be directed to contact ambulance/police/CAT team as necessary.  We do not replace the acute psychiatric services already available.
