4 Year Old Healthy Kid Check
Items 709 or 711 can be provided to children aged 4 years old by the patient’s usual doctor. As part of this consultation the provider which can be either a doctor or a nurse are expected to provide a copy of a booklet called Get Set 4 Life – habits for healthy kids. This booklet is to be distributed by the Department of Health, PGPN has no role it its distribution. To order copies of the Get Set 4 Life guide please send an email to resourcekits@health.gov.au If GPs and Practice Nurses wish to undertake the Healthy Kids Check and do not yet have copies of the 'Get Set 4 Life - habits for healthy kids' Guide, the Check can be undertaken and a copy of the guide provided to the patient's parent or guardian at a later date. 45 - 49 Year Old Health Check
This is a once only Health Check for patients aged 45 -49 years old inclusive who are at risk of developing a chronic disease. The aim of the Health Check is to detect and prevent chronic disease and allow early intervention. Patients must have identifiable risk factors such as: -

Lifestyle –smoking, physical inactivity, poor nutrition, excess alcohol intake

Biomedical- High Cholesterol, BP, Weight

Family history
This Health Check should be conducted by the patients usual doctor. The practice nurse may assist the Doctor to complete the Health Assessment. Consider risk factors opportunistically when seeing patients or using practice data manipulation tools such as the Clinical Audit Tool to identify patients in this age group, use mail invitations to invite patients in for this Health Assessment. Promotion in the waiting room, such as posters and patient information sheets.

Peninsula Hospice Service hold case conferences every Thursday. GPs are invited to participate via the telephone if one of their patients is being discussed.

Cervical Screening
Reminders, recalls and prompts are used to initiate prevention, before or during the patient visit. They can be either proactive or opportunistic. Resources to enable a practice to set up a system to deal efficiently with the pap smear process are contained in the first 2 documents. Also included below is a Clinical Audit Tool cheatsheet to help you to use the CAT to identify which women who have or have not had a pap smear at your practice and those who are due and overdue for their pap smear.

The Divisions Information System or now called ChilliDB for Divisions is a sophisticated database and contacts management system that allows divisional staff to record contacts and manage events, practice visits and outcomes. It can be linked to the national performance indicators. This resource was developed by the division to aid the implementation of this product.
