PGPN offers General Practice support through visits, phone calls or email contact along with the availability of resources to support your patient’s referral to a Lifestyle Modification Programs (LMP) to assist them in making life long changes to prevent the onset of Type 2 diabetes.

Burden of disease

It is estimated that 20% of individuals aged 40 – 49 years will be considered at high risk of Type 2 Diabetes (as defined by an AUSDRISK assessment score of 12 or more).    Lifestyle modification is an effective and proven means of preventing Type 2 Diabetes in up to 58% of participants. Hence, the initiation of specific funds nationally to support the risk screening and referral pathway to a lifestyle modification program for this cohort with the intent of preventing further burden of disease.


Patient eligibility
To be eligible for referral a  patient must be;

aged 40 + (or a person from an Aboriginal or Torres Strait Islander background aged between 15-54 years)

• Scored 12 or more on the AUSDRISK Test at home or with the GP;

• Diabetes needs to be excluded with a fasting BGL  done within the past 12 months at a minimum; and

• Patients aged 40-49 require a health check with their regular GP to initiate the referral process to a local Lifestyle Modification Program.

 

Patients must not have Diabetes or have attended a previous LMP. Referrals are made to the LMP provider directly. The LMP provider will contact the GP on the patients progress.

 

MBS item numbers:
Patients may be eligible for one of a number of health assessments depending on their age and previous history. A summary of MBS item numbers available to support GPs to undertake health promotion & disease prevention activities is available  here.  

Practice Nurses and Registered Aboriginal Health Workers can assist GP’s in performing Health Assessments. The time that a Practice Nurse or Registered Aboriginal Health Worker spends in assisting with a Health Assessment may be added to the time taken by the GP to complete the assessment.
