The Network provides support for general practitioners and their staff by way of two independent software programs, the Practice Health Atlas (PHA) and the Clinical Audit Tool (CAT). These programs can be used in conjunction with each other to support quality improvement in information management as well as enhance your business capabilities; i.e. the programs can be used to map out how well you are doing in your practice, examine specific population sets and examine where and how you could improve your bottom dollar.
The Practice Health Atlas
The Practice Health Atlas works by capturing the practices de-identified data from Medical Director, Best Practice, Genie and Zedmed which is presented to the practice in a succinct and informative report which includes:
• Epidemiology and mapping data - outlines your patients’ epidemiology and demographics over a number of chronic diseases.
• Business and clinical systems modelling information - summarises your MBS item number utilisation which allows you to examine more closely the business side of your practice and ultimately enable you to increase your income.
The PHA can be utilised in at least two ways to benefit your practice:
• As an information resource; Compare with the GP / Practice’s existing knowledge.
• For business and clinical systems modelling / re-design; Complete discussion points for topics raised in each section.
Once we have conducted the PHA on your practice this information is collated and can be presented as a whole of practice session looking at individual staff roles and where improvements can be made to work more efficiently as a team.
The Clinical Audit Tool
The Clinical Audit Tool can be installed directly onto desktop computers and works in collaboration with the GP Clinical desktop system (MD, Zedmed, Best Practice and Genie) at the practice. The CAT can be used by practice staff to profile their patient population. The CAT tool works by taking a ‘snapshot’ of your clinical data and presents it in a more meaningful manner in either pie, bar graphs or patient lists.
The tool;
• The tool allows practice staff to work towards data improvement by assessing the quality and completeness of their patient information which can help with meeting practice standards as well as for accreditation purposes.
• The CAT can also help staff to better manage their patient’s chronic diseases by identifying needs and prioritising patients for disease management initiatives.
• The CAT allows you to focus on specific groups within your population by allowing you to simply filter the population based on your specified criteria.
• The data can be used to drive informed whole practice decisions and population health initiatives.
Ongoing training can be provided if required and we can demonstrate to you the many ways by which practice staff can use the CAT to meet patient’s health needs, as well as focusing on the practices areas of specialities and interests.
