FMPML- Have your say in the Health Needs Assessment

One of the first tasks of the FMPML is to prepare a Primary Health Care Health Needs Assessment for the communities of our catchment. You are invited to tell us about the major health needs that you see as priorities for the catchment, the gaps in local primary health systems and services – what is working well, and what doesn't work well – and opportunities that would improve the health outcomes of your population.

Peninsula GP Network identifies practice nurses as integral to the general practice team.
As a support organisation, PGPN wishes to obtain your thoughts on your current needs as a nurse working in general practice on the Mornington Peninsula. Information received will be beneficial in assisting us to offer support.

We would appreciate if you could take the time to fill in the Practice Nurse Survey located on the PGPN Practice Nurse Page. Your reply will also be placed in a draw for a gift of a $50 Myer voucher, and will be drawn on the 29th February 2012. The winner will be contacted by phone and the result will be published on the website and our Facebook and Twitter pages.

» Mapping of existing after hours services within our region
A key role for Frankston Mornington Peninsula Medicare Local is to conduct capacity mapping of existing after hours services within our region. Develop an understanding of the afterhours primary health care needs of our local community, and to address gaps in local afterhours health service provision.
With the above in mind please download the data gathering template to be completed by each practice to enable us to gather as much information as possible about after hours services in our region. Could you please complete the attached template and return to Jackie Bange via email or fax 9708 8157. Following on from this Jackie Bange will be contacting practices the week commencing January 31st to meet with you, to discuss this further.

Asthma Foundation Survey

The Asthma Foundation of Victoria is conducting a Community Asthma Services Mapping Project in Victoria. The aim of project is to identify current services and organisations providing asthma support, advice and education to people living with asthma in Victoria. It also aims to identify any gaps in asthma services in the state for future planning and health service delivery.

 

Please complete the brief survey that is designed to identify current community asthma and respiratory clinics, pulmonary rehabilitation programs, asthma support groups, and asthma educators in Victoria. The survey should take only 5-10 minutes to complete and will be available to be completed until the 29th February 2012.

Perinatal Survey
The National Perinatal Depression Initiative aims to improve prevention and early detection of antenatal and postnatal depression, and to provide better care, support and treatment for expectant and new mothers experiencing depression. The initiative has provided national guidelines for the routine screening for perinatal depression and workforce training and development of health professionals. This survey is a follow up to the survey distributed in late 2010. It aims to provide the Frankston Mornington Peninsula Perinatal Mental Health Partnership Group with information about the current skill base of practitioners in the assessment and treatment of perinatal mental health and knowledge of available resources and services.

Your assistance in completing this short survey is greatly appreciated. Results will remain anonymous and only collated results will be published. Survey closes on 5th March 2012.

» ATAPS Referral Update - Peninsula GP Network will require ATAPS referrals to include a Mental Health Treatment Plan and Referral Form. The referral form will allow us to maintain more accurate records for reporting and ensure referrals are handled promptly. The referral form will be emailed to all Practice Managers for distribution and is also available on our website resources page in both PDF and word formats.
