PGPN will support general practice through the promotion and provision of education to the whole of practice on how best to utilise the Practice Health Atlas (PHA) and Clinical Audit Tool by:
• Assisting practices to understand the purpose of and how best to use the PHA and Clinical Audit Tool software.
• Promote to practices the benefits of developing systems to support quality clinical improvement by provision of resources and direct GP/practice contact focusing on diabetes, cardiovascular disease, cancer and respiratory illness prevention and early interventions and evidence based practice.
PGPN will encourage practices to share de-identified data to:
• Benchmark data across practices
• Benchmark data within practices
• Improve the quality of clinical outcomes in our local population in prevention and management of chronic illness
PGPN aims to build the capacity of general practice nursing through;
• Coordination of activities which facilitate the promotion and development of nurse coordinated preventative health and chronic disease management clinics
• Support and facilitation of a Practice Nurse Network incorporating opportunities for clinical skill development, peer support and nurse mentoring
• Conduct a needs assessment in practices with nurses to identify trends and priorities for targeted capacity building activities
• Re-initiation of practice nurse clinical attachment package for chronic condition care supporting enhanced clinical management, care coordination and service access to enhance the patient journey and outcomes.
Education programs developed for GPs and staff in 2011-12 will specifically focus on;
• Preventative health and risk factor screening
• Population health needs and data management
• Chronic disease management
• Care coordination and self management
Frankston & Mornington Peninsula Smoke Free Charter
Action is needed to reduce the harm caused by smoking. Tobacco is a major cause of preventable death across Frankston & Mornington Peninsula. PGPN is committed to reducing the prevalence of smoking and protecting people from the damaging effects of second hand smoke.
