Life! Taking Action on Diabetes
The Victorian Government has committed funding until 2015 to deliver the Life! Diabetes and Cardiovascular Disease Prevention Program. This program offers a six session course over eight months aimed at improving the participant's physical activity and dietary habits over the long-term.
The program also includes social marketing to increase community awareness of the importance of physical activity and dietary habits to reduce the risks of diabetes and cardiovascular disease. The initiative contributes to the Department of Health's Health Advancement output.
The Life! Program is suitable for:
• Victorians at high risk of Type 2 Diabetes Mellitus (T2DM) (AUSDRISK score 12 or more) aged 50 years and over who do not have diabetes
• Aboriginal Victorians aged 18 years and over (AUSDRISK score 12 or more) who do not have diabetes
• Victorians aged 18 years and over who have a history of Gestational Diabetes Mellitus (GDM) or Ischemic Heart Disease (IHD) and have not had a diagnosis of diabetes
• Victorians aged 18 years and over who have had a WorkHealth check and were identified at high risk (AUSDRISK score 12 or more) who do not have diabetes

Please note: Lifestyle Modification Program
As of the 1st November 2011 the funding to support patients aged 40-49 to attend Lifestyle Modification Programs has ceased. Patients' already attending sessions have until April 2012 to complete their sessions. Local facilitators will continue to offer programs at a cost to the patient.
